
Teams from CR3 and ZymeFlow have recently completed DNV SCAT training, an investment in the skills that underpin how we identify, investigate and learn from incidents.
SCAT, the Systematic Cause Analysis Technique, is a structured method developed by DNV for investigating incidents. It gives teams a consistent framework to work through what happened, trace the immediate causes back to underlying and systemic factors, and identify the controls needed to prevent it from happening again.
Completing the programme has strengthened our ability to run structured investigations, pinpoint root causes, and put corrective actions in place that address the real issues rather than the surface ones. It sharpens how our people approach safety and operational performance.
